Pretty neat!
By default on my VS2010 it collapses some stuff (highly nested blocks?).

Maybe it shouldn't collapse anything on the first run. Just an idea.

It currently collapses by default everything that is started after 8th column. Visual Studio does not automatically provide even "Collapse All" command. I will change default behaviour as soon as I'll add some collapsing commands.

2 minor issues I noticed, both related to the block highlighting that happens when one hovers the [-] boxes:

1) I use bright text on dark background. The highlighting uses a bright colour that makes text unreadable. Is that using one of the standard colours that can be changed from the options? If not, maybe you could ensure you have contrast with the text colour.

2) Sometimes (possibly when scrolling is involved) a block of text remains highlighted even if the cursor is outside the [-] box. Hovering over a box fixes it.
I don't know about the VS extensions architecture, but you may want to do the highlighting test not only on mouse move, but also on other events that may place the cursor outside the [-] box (like scrolling, main window losing focus, tab switch, ...). <- Over-the-Phone-Debugging :-)
